Any child who will be 5 years old by September 1, 2016 is invited to attend the kindergarten screening February 22nd and 23rd.
Kindergarten Readiness Survey February 22nd

The Kindergarten Readiness Survey for all 2016 – 2017 kindergarten students
will be held at Madison Elementary School:
February 22nd: 7:30 a.m. to 6:30 p.m.
February 23rd: 7:30 a.m. to 10:30 a.m.
The survey will take approximately one hour with paperwork for parents to fill out.
Parents are asked to park in the North Parking Lot during the survey.
Parents are asked call the school for an appointment on one of the following dates:
February 18th: 8:00 a.m. to 3:00 p.m.
February 19th: 8:00 a.m. to 3:00 p.m.
PLEASE DO NOT MAKE EARLY CALLS FOR AN APPOINTMENT
To be eligible for kindergarten a child must be five years old on or before
September 1, 2016. A child who is six years old on or before the first day of
September is required to attend school in accordance with the compulsory
attendance law.
In order to register for kindergarten you will need to bring with you:
- A copy of your child’s CERTIFIED BIRTH CERTIFICATE
- Current IMMUNIZATION record on the long form
State law requires that the school have a copy of the shot record and birth
certificate on file. Hospital certificates cannot be accepted.
School district officials ask that if you know of someone in your neighborhood
that needs to be notified about the survey, please inform them. Those with questions are asked to call 256-7721.
