Madison Community Hospital Survey

First District of Local Governments is assisting the Madison Community Hospital by conducting a survey and they would like your input.

The survey asks participants their thoughts on what services are most important to them; whether they have any specific healthcare needs; and why they may not presently use the Madison Community Hospital. The survey also asks what ways the hospital serves the community well and if there are any specific needs that aren’t being met. Survey results will allow the hospital to plan for future services in the community.
